*Extended access to McKinley Health Center and Counseling Center services is available. To be eligible for the Extension Fee, you must have been enrolled the previous semester and paid the Health Service Fee.

**To purchase Graduate Spousal Fee you need to provide copies of the following documents.
Note: This allows access only to McKinley Health Center, not the Counseling Center.

• Proof of major medical health insurance for spouse or domestic partner
• Marriage Certificate, Civil Union Certificate, Affidavit of Domestic Partnership, or any 3 of the following:
  - Joint mortgage or lease
  - Beneficiary designation for life insurance or living will
  - Power of attorney
  - Joint ownership of a motor vehicle, checking account, or credit card
